Transitional Services

Once clients have progressed through the residential program, they will enter the transitional phase of the program. During the transitional phase of the program, the client may decrease daily involvement in treatment activities and increase outside activities. Outside activities include employment and job searching for preparation to re-enter the community. This phase of the program equates to ASAM PPC Level III.1 Clinically Managed Low-Intensity Residential. Transitional level of care provides the client with residential care and reduced clinical services. Clinical services are similar to the Level II.1 Intensive Outpatient level of care where clients are required to attend at least 9 hours of clinical programming per week. During this time in the client’s treatment, case management services may be increased based on need. The case manager will work with the client to assist in transitioning back into the community and ensuring continuing care services are available.
